Prostate cancer is rated under 38 CFR 4.115b, Diagnostic Code 7528, Malignant Neoplasms of the Genitourinary System. While the cancer is active or being actively treated (surgery, radiation, chemotherapy, hormone therapy), VA assigns a 100 percent rating. This is a mandatory 100 percent rating during that period, not something you have to prove residual symptoms for.
Once treatment ends, VA does not simply keep the 100 percent rating indefinitely. The regulation requires that six months after the cessation of treatment, VA schedule a new Compensation and Pension examination to determine whether there is a recurrence or persistence of the disease. If there is no recurrence, VA rates the condition based on residual symptoms and complications from the surgery or treatment, most commonly under the criteria for voiding dysfunction or renal dysfunction found in 38 CFR 4.115a. These residual ratings run from 0 percent up to 100 percent, depending on the severity of urinary leakage, urgency, frequency, need for absorbent pads or catheterization, or kidney impairment.

Analyze my claim free →A 0 percent rating after this six-month reevaluation window means VA's examiner found no compensable residuals under those voiding or renal dysfunction criteria. It does not mean your cancer history or removal surgery is erased from your record. It simply reflects that, at the time of the exam, your urinary and kidney function did not meet the threshold for a higher rating. If your symptoms later worsen, such as increased incontinence or urgency, you can file a claim for an increased rating at that time, since disability ratings can be adjusted up or down as your condition changes.
It is also worth remembering that a 0 percent rating still establishes service connection, which matters for eligibility for other benefits, presumptive conditions tied to prostate cancer, and any future increase claims tied to residuals or secondary conditions like erectile dysfunction or depression.
If you believe your current urinary or other residual symptoms are more severe than reflected in your 0 percent rating, the concrete next step is to file a claim for an increased evaluation using VA Form 20-0995 (Supplemental Claim) with updated medical evidence documenting your current voiding dysfunction, or request a new C&P exam that accurately reflects your day-to-day symptoms.
