A Compensation and Pension (C&P) examination is scheduled to evaluate the specific condition(s) you have already claimed, or to assess whether an already-claimed condition is connected to something else (a secondary relationship). Examiners do not have authority to open a new claim on your behalf, even if they note during testing that you show signs of another condition, such as carpal tunnel syndrome. Their job is to document clinical findings and give a medical opinion; the report goes into your file, but VA's adjudication system will only rate what has actually been claimed. If carpal tunnel was not part of your original claim, noting it in the exam report does not automatically put it into the system as a claim VA will decide.
To get that condition considered, you need to file a claim for it. Since this would be a new issue rather than a request to reconsider a denied one, the correct form is VA Form 21-526EZ (Application for Disability Compensation and Related Compensation Benefits). If you are not ready to submit the full claim right away, you can file an Intent to File (VA Form 21-0966) to lock in an effective date while you gather evidence, such as buddy statements or medical records showing when symptoms began in service.

Analyze my claim free →When you file, reference the C&P exam and the examiner's notation, since that report is already part of your record and can serve as favorable evidence, especially the finding that testing suggested carpal tunnel. If the numbness and dexterity loss were already mentioned in connection with another claimed condition, it may also be worth asking whether carpal tunnel could be considered secondary to that condition, which uses the same 21-526EZ but should include a statement addressing the secondary connection.
Do not assume the exam report by itself will result in a rating decision on carpal tunnel; without a formal claim, VA has no pending issue to adjudicate for that condition, and it could be missed entirely.
The concrete next step is to submit VA Form 21-526EZ (or at minimum an Intent to File) specifically listing carpal tunnel syndrome as a new claim, referencing the C&P exam findings as supporting evidence.
