How do I document possible carpal tunnel syndrome for a VA disability claim, especially if it's related to an already service-connected condition?

Carpal tunnel syndrome (CTS) is a medical diagnosis that generally requires objective testing, most commonly a nerve conduction study (NCS) and/or electromyography (EMG), to confirm median nerve involvement. A primary care visit is a reasonable first step, but if symptoms are severe, urgent care or an orthopedic/neurology referral can get testing started sooner rather than waiting weeks. Getting the diagnosis on record matters more for a future claim than which doctor makes it.

For VA purposes, there are two main paths to service connection. Direct service connection requires evidence that the condition began during or was caused by military service, which is unlikely if it stems purely from a current civilian desk job. Secondary service connection, under 38 CFR 3.310, is more relevant here if the wrist/hand symptoms are being caused or aggravated by an already service-connected thumb condition (for example, altered grip or wrist mechanics from compensating for thumb pain). To support a secondary claim, a treating provider would ideally state, in writing, that the CTS is at least as likely as not caused or aggravated by the service-connected thumb condition — this nexus opinion is often the deciding factor.

Documentation that helps any claim includes: dated symptom notes (onset, frequency, severity, what triggers it), copies of work restrictions or accommodations (like needing voice-to-text or reduced typing), the brace prescription, medication records (such as meloxicam), and any NCS/EMG results once completed. If a nexus opinion isn't available from the treating physician, a Disability Benefits Questionnaire (DBQ) completed during a VA Compensation & Pension exam can also address causation once a claim is filed.

When ready to file, VA Form 21-526EZ is used for an original or secondary claim, or supplemental claims with new evidence can be filed using VA Form 20-0995. Ratings for peripheral nerve conditions like CTS fall under 38 CFR 4.124a, based on whether the median nerve involvement is mild, moderate, or severe (incomplete paralysis) or complete paralysis, and whether it affects the major or minor hand.

The concrete next step is to keep the primary care appointment but push for an EMG/NCS referral at that visit, and separately ask the provider to document whether the current symptoms could be related to the existing service-connected thumb condition.
