Filing a new claim for an unrelated condition does not automatically trigger a review of your previously established ratings. The VA generally processes a claim based on the specific issues you list on your application (VA Form 21-526EZ). A Compensation and Pension (C&P) exam scheduled for a new claim is typically limited in scope to the condition being claimed. For a scar claim, the examiner would assess the scar under the relevant diagnostic codes in 38 CFR § 4.118, not your other service-connected conditions.
That said, there are limited circumstances where existing ratings can come under scrutiny. If a condition is rated at a level requiring a future reexamination (sometimes noted in your rating decision as a scheduled reexamination), that review happens independently of any new claim you file. Separately, if information submitted with your new claim happens to contain evidence that directly contradicts the basis for an existing rating, a rater could, in theory, flag it for further development. This is uncommon for conditions that are truly unrelated, but it is not impossible if, for example, new medical records inadvertently reveal improvement in an unrelated system that happens to be rated.

Analyze my claim free →It's also worth knowing that most service-connected ratings become protected after a certain period. Under 38 CFR § 3.951 and § 3.957, ratings in effect for 20 years are protected from reduction except in cases of fraud, and ratings in effect for 10 years cannot be severed for service connection except for fraud. Ratings in place for fewer than 5 years face more scrutiny if a future exam shows material improvement, but that scrutiny arises from scheduled reexaminations, not from filing an unrelated new claim.
In practice, thousands of veterans file new, unrelated claims each year without any impact on their existing ratings. The key is making sure your new claim is cleanly presented as a separate issue, with medical evidence specific to that condition, so the rater has no reason to look beyond the scope of what you're claiming.
The concrete next step is to file the claim for the scar using VA Form 21-526EZ, clearly listing it as a new and separate condition with supporting medical evidence (photos, treatment records, nexus statement if needed), and attend the scheduled C&P exam, which should focus solely on the scar itself.
